Send keep-alive messages in streaming for long-running requests

In case of, for example, an agent with a tool call that is long running (e.g. more than 60 seconds), sometimes we hit network idle timeouts and it would be good to be able to send a keep alive message every N seconds, if no event comes from the agent.

Hey @antonioalegria — agreed, this is a real problem for long tool calls behind proxies that idle-timeout the SSE connection. There's an attempt in #299 (heartbeat : ping comments while the agent is quiet) — I'd be happy to take a clean, tested version of that. Thanks for the 👍 nudge.
