feat: add PCD entity history

• Fetch PCD repos with a blobless git clone instead of a tarball, mapping each op file to its commit with one git log.
• Replay op files one at a time during compile:pcd and record per-entity history, asserting at the end that the replayed state equals the compiled state. --no-history skips it.
• Add a History section to every PCD entity page: one row per commit, expandable to readable change summaries with links to related entities. Markdown artifacts get a ## History table.
• Present changes with per-shape presenters (scoring, conditions, patterns, descriptions, qualities, tags, tiers) and a YAML diff fallback.
• Widen the content column on PCD routes, add a locale-aware numeric DateTime format, let expandable table rows toggle on row click, and reset expansion when table data changes.
