Stop recursive gates and enforce incident cleanup - #3

…evidence log Description now leads with apparatus creation/retention instead of 'non-trivial software work' (trigger battery: 12/12 positives kept, 12 clear false fires eliminated, ordinary-task co-firing 30/30 -> 19/30). 'Tests are machinery' gains the retention criterion (would this proof alone catch a real recurrence - never age or origin) after two observed over-prunings of a live rule's only proof; 2/2 re-runs then retained it. README rewritten in plain English with a keep/delete example table; docs/testing.md records the evidence and gaps. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>

What changed
Why
The previous rules allowed release gates to self-justify, allowed comments to count as expiry mechanisms, and did not force an after-incident subtraction pass.
Impact
This is an additive instruction change. Skill discovery, invocation policy, and packaging are unchanged. Rollback is a one-commit revert.
