feat(handover): sync human messages to agent history

Summary

• add the optional session_history extension with separate user and assistant slash commands
• route respond.io messages by assignee and sender source without recording Momo replies twice
• record through the normal agent turn path so records and turns share the conversation lock
• document setup, routing, diagnosis, and limits

• added tests first for configuration, ownership, routing, recording, failure logs, and per-conversation exclusion
• make check
